Options for Accessing Your Records

System Health Information Management maintains records of the care patients receive while they are at Rancho Springs Medical Center. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization from the patient.

Retrieval Fees and Costs

All individuals and organizations who request medical records will be responsible for the costs to produce the requested health information records. It takes time, equipment and people to create a copy or reproduction of these documents.

The fee charged to defray the costs is regulated by California Health and Safety Code 123100. The fee is calculated as twenty-five cents ($0.25) per page, reasonable clerical costs of $24.00 per hour billed in quarter hour increments, actual shipping charges if the records are mailed and any applicable California State Sales Tax.

Please contact us if you would like an estimate of the charges or assistance in determining the records you need before making your request. We can help you make sure you get the exact records you need and are not charged for anything you do not require.

After we have processed your request for medical records, we will mail the information to the address specified on your request form, or you may pick up your copy from our office. Due to the large volume of requests we receive, it may take up to 5 business days to fulfill your request from the date we receive it.

You may visit the Centralized Release of Information Department to deliver your written request in person. The department is open from 8:30 a.m. to 5:00 p.m., Monday through Friday, excluding national holidays. The Centralized Release of Information Department is located at 25485 Medical Center Drive, Suite 106, Murrieta, CA 92562.

Radiology Images and Studies require one full business day to prepare. If you are scheduling a follow-up appointment please plan accordingly.
